Please use this ballot to vote on the two issues below no later than
Monday, December 1, 2008.
Issue 1: Approval/Disapproval of the proposed budget for 2009 (copy attached). In accordance with the Avondale Declaration of Covenants, Restrictions and Easements, owners must vote on whether to accept or reject the attached budget.

Issue 2: Election of Board Members for the period 1/1/09 through 12/31/09. In accordance with the Avondale Declaration of Covenants, Restrictions and Easements, please vote for AT LEAST FIVE (5) of the following individuals to serve as your board of directors. The resulting Board of Directors will then elect the Executive Officers.
